Qualifications are due on Friday, November 12th, 2021 by 5PM. In order to upload your submission, you will need to enter the following information below:- the Name of the lead applicant organization
- the lead applicant organization’s Federal Employer ID Number (FEIN)
- the lead applicant organization’s Department of Unemployment Assistance ID Number (DUA ID)
- the Total Program Design Funds Requested
- the lead applicant organization’s Primary Contact Person (to be notified upon decision of grant award)
Please review all components prior to uploading in order to ensure you have completed all of the required information. Please note the required naming conventions when you save and upload your attachments in the following file formats:
- Qualifications forms and charts must be submitted in the form of a single MS Word file (Attachments 1, 2, 3 & 6)
- Budget forms (including Outcome Chart) must be submitted in the form of a single MS Excel file (Attachments 5a, 5b and 5c)
- Signed MOAs and/or letters of commitment must be scanned and submitted in the form of a single PDF file (Attachment 4)
- C.O.G.S. must be submitted in the form of a single PDF file (Attachment 7) *Please note that community colleges, public vocational technical high schools, and public state universities serving as the lead applicant do not need to submit a Certificate of Good Standing.
